Payment Options for Your Eye Care

It is the patient’s responsibility to be both aware of and understand his or her health and vision insurance coverage. In an effort to keep costs down, our practice bills for services at the time they are provided. Payment for any co-insurance, deductible, or co-payments will be collected at the time of services. If special arrangements need to be made to make an appointment with us, please contact the office manager prior to your visit to make said arrangements.

Our practice offers discounts on services that are not covered by insurance or for people who are uninsured. These discounts will not be applied to any services covered by insurance.

Refractions are a necessary component for many exams to determine a patient’s eyeglasses or contact lens prescription. Refractions are also necessary when patients have optical medical problem, when they have surgical/laser procedures performed, or when they are taking some medications. Some insurance companies do not cover this service, in which case the patient is responsible for paying the $20.00 fee. Medicare, Choices Plus, Humana, Humana Gold Plus, and Medicaid are a few that DO NOT cover the refraction fee.

For your convenience we accept the follow methods of payment: Cash, Checks, Master Card, Visa, Discover, American Express and Debit.

Please bring your insurance card with you to every visit. Our staff will verify coverage before services are rendered.
